REMINDER
WHETHER YOU PAID FOR THIS TRAINING OR NOT, THE TERMS OF USE YOU CLICKED THAT
YOU AGREED TO, BINDS YOU TO THESE TERMS:
IF YOU DO NOT AGREE, DO NOT PROCEED WITH TRAINING! Call us for a partial refund, (90% if
less than 20% has been completed). ONCE YOU HAVE COMPLETED MORE THAN 20% OF THE
TRAINING, NO REFUNDS WILL BE PROVIDED.
1. ALL CLASSES ARE SINGLE-USE UNLESS OTHERWISE STATED. DO NOT PROJECT
THE TRAINING FOR A GROUP, DO NOT SHARE YOUR COMPUTER SCREEN WITH
ANYONE ELSE SEEKING TRAINING. IF YOU HAVE QUESTIONS ABOUT HOW TO
ARRANGE THIS TYPE OF TRAINING CLASS, CONTACT US AT 719-502-7081.
You HAVE ABSOLUTELY NO RIGHT(S) AND AGREE TO NOT REPRODUCE, COPY
(INCLUDING SCREENSHOTS), PRESENT, REPUBLISH AND/OR TO DISTRIBUTE THE
TRAINING CONTENT, OR PORTIONS THEREOF. STUDENT SHALL HAVE ABSOLUTELY
NO RIGHT(S) TO LICENSE OR SUB-LICENSE TO THIRD PARTIES, NOR INVITE
ADDITIONAL VIEWERS TO HIS/HER CLASS(ES).
2. PAYMENT ACKNOWLEDGEMENT AND REFUND POLICY: Use of a credit card not in your
name verifies you were provided with full permission to use the card from the cardholder for
the purpose of purchasing a course(s) in your name. Using a card without permission will void
certification and criminal charges will be filed against you upon receipt of a complaint by the
cardholder or financial institution, and we will fully cooperate with any requests received to
further criminal charges.
Furthermore, Your account profile must be set up in the name of the student taking the
class. If you used someone else's card to pay, please click on the “Details” tab in your profile
to be sure the name is the student who is taking the class is listed (not the cardholder). Do
this prior to completing any classes! Failure to do so will result in the Certificate being in the
cardholder's name!
3. You agree to refer any requests for training by others to Integrity Medical Courier Training by
calling 719-502-7081.
4. Certification of compliance is not effective until the Training Program has been paid in full and
satisfactorily completed.
5. Certification of compliance will expire one-year after the satisfactory completion of the
Training Program. Once you’ve completed more than 20% of any course, cancellation of
payment immediately voids certification and could result in a criminal complaint against you.
6. You understand that Ken Arnold and Integrity Medical Courier Training, and/or any additional
and/or applicable authors or instructors, assume no civil or criminal liability for service(s)
rendered by you, and/or failure of service(s), including but not limited to product and/or
specimen integrity, HIPAA compliance, loss of product, safety of employee(s), independent
contractor(s), and/or patient(s), Courier clients and their workforce, and/or community.
7. Student “hereby indemnifies and shall hold Instructors, Authors and/or Program Partners, its
associates, licensees, distributors, successors, and assigns, free and harmless from any and
all loss, damage, or expense, including attorneys’ fees, resulting from any litigation, claim or
proceeding arising out of a breach or an alleged breach of any warranty, covenant or
representation of the Courier contained in this Agreement."
8. These terms apply to all references to training, training program, classes, and/or
presentations conducted by Integrity Medical Courier Training, our instructors, and/or
affiliates.
9. You understand that although specific information/content in our training program(s) may not
be proprietary in its own right, its compilation is and will be treated as copyrighted with all
rights reserved. You understand training content may contain the trade names or trademarks
of various third parties, and if so, any such use is solely for illustrative purposes only. All
product and company names are trademarks™ or registered® trademarks of their respective
holders. Use of them does not imply any affiliation with endorsement by, or association of any

10. PAYMENT ACKNOWLEDGEMENT AND REFUND POLICY
NO REFUNDS will be made for presentations you viewed and/or attended more than 20%.
